In Jackson, drive east past the square. Half a mile down, pass the hospital on the left. 640 E. Broadway is a brown building on the right just after the hospital.

How it Works

Your first visit is called an Intake/Assessment: a counselor will ask you a series of questions about your personal history and circumstances so that we can assign you a therapist to meet your needs.  Once assigned, your therapist will call to arrange your first counseling appointment.

Walk-Ins

We offer walk-in appointments for those that need to speak with a therapist that day. Those services are available during office hours M-F 8am to 5pm.  For admissions/intakes, please call our office and they can let you know what walk-in intake appointments are available that day or the next.

What to Bring

Bring your driver’s license, insurance/Medicaid/Medicare card (if applicable), and proof of income with you to your first appointment. Your proof of income can be a copy of your pay stub, first page of your tax return showing adjusted gross income, or W-2. You will be charged our full standard fee until an application for the sliding scale fee is completed and proof of income is provided. Income verification is needed for our sliding fee as well as to determine if you may be eligible for state funded services. Those who are not eligible for state funding may then apply for the sliding fee but we must have income verification. You will be charged full fee until we have income verification or you have applied for state coverage.

Parents

You will be asked to sign a consent form. Any custody papers/issues must be disclosed.

Confidentiality and Privacy

We value your privacy and are bound to it by law. We do not disclose client names, appointment times, or any other information without written authorization from you, except in certain situations required by law.

Cancellations

You must cancel your appointment more than 24 hours prior to your scheduled appointment time. If you cancel within 24 hours of your appointment, or have 3 or more times that you no show for a scheduled appointment, you may lose the privilege of the same time slot and it could result in a reset in requested fee adjustments.

Counseling Fees

Our standard fee is $210 hourly for therapy. We can bill private insurance, Medicaid and Medicare (see below). Payment is due at the time of your appointment.

Sliding Scale Fees: Available for those who do not qualify for Behavioral Health Coverage from the state and is based on income.

Please provide proof of income (pay stub, tax return, or W-2) to be eligible for this program.

Insurance: Your insurance policy may cover some mental health services. We bill insurance at our full rates. You may be required to pay an insurance deductible and/or co-payment, depending on your policy. If you are on a sliding scale fee, deductibles, co-payments, and services not covered by third-party payments will not exceed your sliding scale fee.

Change of insurance: It is your obligation to inform the receptionist if you have insurance that you wish to use for billing and when there are changes to your insurance status.

Medicaid or Medicare: If you are covered by Medicaid or Medicare, you will not be charged a copayment but may be charged on the sliding fee scale, for services not covered by those programs. The Children’s Healthcare Insurance Program (CHIP) under Medicaid requires a co-pay that you are responsible for.

Preauthorization: If your health insurance requires pre-authorization for services, it is your responsibility to coordinate this with your insurance company. If preauthorization is pending and you are on a sliding scale fee, we will charge you the full sliding scale fee until all preauthorization requirements are satisfied.

Third party contracts (DFS, DVR, EAP, etc.): Until verification is received, you will be responsible for the standard fee or your sliding scale fee.

Payment for minors: Parents are responsible for fees associated with treatment of their children. The parent or guardian signing the document will be considered the responsible party unless court documents are producted within 10 days of service indicating otherwise.

Evaluation and court-ordered assessments are not subject to SSF rates and may cost more. Ask for more information about these rates.

Overdue accounts may be sent to a collection agency.
